An EPSDT well-child visit is defined by five components: a comprehensive health and developmental history, an unclothed physical exam, age-appropriate immunizations, laboratory testing including blood-lead screening, and health education and anticipatory guidance.
Beyond the regular schedule, Medicaid requires coverage of interperiodic screening visits when medically necessary, such as when a child needs diagnosis or treatment of a condition that was not present at the last well-child visit.
Federal law does not write the periodicity schedule itself. States must build their schedules on current pediatric medical, vision, hearing and dental practice standards, so the expected screening frequency can differ by state.
Under EPSDT, a covered Medicaid service or supply must be available to a child when it is medically necessary to correct or ameliorate the condition, even if the state plan does not list it for children generally.
VFC vaccines are supplied at no charge to eligible children. This does not mean the vaccine product code is omitted from every claim. Check the state Medicaid or managed-care instructions, required modifiers and administration-code rules for the date of service.
