What changed
CMN and DIF forms are no longer the current claim workflow
For dates of service on or after January 1, 2023, CMS discontinued Certificates of Medical Necessity and DME Information Forms. Sending those forms—or their electronic data elements—with a current claim may cause the claim to be rejected and returned.
The change did not remove the supplier's documentation responsibility. A compliant file still needs the applicable Standard Written Order, medical-record support, correct coding and proof of delivery. Item-specific LCD, policy article, prior-authorization and payer requirements can add further checks.
